    Yo, un Invitado adulto o un padre (que acepta en nombre de un menor o pupilo que es un Invitado) acepto y asumo expresamente todos los riesgos de lesión, enfermedad, daños o muerte, conocidos y desconocidos, inherentes o no, por mi uso y participación en todos y cada uno de los servicios, comodidades, instalaciones, maquinaria, equipo, vestuarios, estacionamiento, lobby y cualquier otra área ubicada dentro y alrededor de las instalaciones del Club, independientemente de si dicha lesión o daño surge de o durante la actividad física, incluyendo, entre otros, cualquier pérdida o robo de cualquier propiedad personal. Soy plenamente consciente de que el funcionamiento de las instalaciones del gimnasio y la participación en actividades físicas pueden ser peligrosos e implican riesgo de lesiones para mi persona. Acepto que tengo el deber de examinar cada pieza de equipo o instalación antes de su uso, y abstenerme de usarlo e informarlo al Club si hay algún indicio de que dicho equipo o instalación ha sido sometido a abuso, está dañado o se encuentra en una condición insegura o potencialmente peligrosa. Si soy padre de un Invitado menor, he discutido las actividades, responsabilidades y riesgos con el menor, quien los comprende y acepta, y yo, en la máxima medida permitida por la ley, los acepto en nombre de dicho menor.

    Acepto, en mi nombre (o el de mi hijo menor o tutelado), mi cónyuge, hijos, herederos y representantes personales, renunciar, liberar y despedir para siempre al Club y a sus propietarios, empleados, afiliados, sucesores y cesionarios de todos y cada uno de los reclamos o causas de acción, conocidos o desconocidos, que surjan de todas y cada una de las lesiones o daños causados por la negligencia ordinaria del Club o sus empleados, propietarios o afiliados, incluidos, entre otros, cualquier reclamo, daño o lesión que pueda ocurrir como como resultado de cualquier mal funcionamiento o rotura del equipo, el mantenimiento inadecuado por parte del Club de cualquier maquinaria, equipo o instalaciones, la instrucción o supervisión negligente del Club y cualquier lesión por resbalones o caídas mientras se encuentre en las instalaciones del Club o en cualquier parte de las instalaciones por cualquier motivo. Acepto expresamente indemnizar y eximir al Club de cualquier pérdida, responsabilidad, daño o costo en el que el Club pueda incurrir debido a mi presencia en las instalaciones del Club.

    AL COMPLETAR ESTE FORMULARIO Y FIRMAR/INICIAL A CONTINUACIÓN, RECONOZCO QUE HE LEÍDO DETENIDAMENTE Y ENTIENDO ESTA RENUNCIA DE RESPONSABILIDAD Y ASUNCIÓN DE RIESGOS Y ENTIENDO COMPLETAMENTE QUE ESTOY RENUNCIANDO A MI DERECHO A HACER UN RECLAMO O PRESENTAR UNA DEMANDA CONTRA EL CLUB POR DAÑOS PERSONALES , DAÑOS A LA PROPIEDAD, MUERTE POR NEGLIGENCIA, RESPONSABILIDAD POR PRODUCTOS O BAJO CUALQUIER OTRA TEORÍA, EN LA MEDIDA MÁXIMA PERMITIDA POR LA LEY.
